EFFICACY

効果効能

本剤の有効成分「セマグルチド」は、GLP-1というホルモンに作用する薬剤で、
以下のような作用を示します。

血糖値を安定させる作用

インスリンの分泌を助け、グルカゴンの分泌を抑えることで、食後の急激な血糖上昇を防ぎます。血糖値が安定することで、余分な糖が脂肪に変換されにくくなり、脂肪の蓄積を防ぐ効果が期待されます。

食欲を抑える作用

脳（視床下部）に働きかけ、自然に食欲を減らします。過剰な摂取カロリーを防ぐことができ、無理なく摂取量をコントロールできます。

胃の動きをゆるやかにする作用

食べたものが胃からゆっくり移動するようになるため、満腹感が持続します。間食や過食を抑えることで、継続的なカロリー制限につながります。

※なお、日本国内においては、本剤は2型糖尿病治療薬としてのみ承認されています。

医学的根拠

オゼンピック（セマグルチド）を対象とした国際共同研究（SUSTAIN1）では、
・HbA1c（血糖値の指標）が約1.5%改善
・体重が平均3.7~4.5kg減少
という結果が報告されています。
副作用としては悪心や下痢などの消化器症状がみられましたが、いずれも多くは軽度～中等度で、安全性も確認されています（Sorli et al., 2017）。

HOW TO TAKE

服用方法

  • 容量
    ・定められた用量（0.25mg〜1mg）を注射してください。
  • 頻度
    ・週に1回、必ず決められた曜日に打つようにしてください。
    ・オゼンピックは食事の影響を受けない薬であるため、投与時間は朝・昼・夜のいつでも、また食事の前後を問わず可能です。
  • 服用方法
    ・注射する部位 以下のいずれかの脂肪組織が豊富な部位に自己注射してください。 太もも
    ・上腕部
    ・腹部
    ・注射部位は毎回2〜3cmずつ位置をずらしてください。 同じ箇所への繰り返しの注射は、皮膚の硬化を引き起こす可能性があります。
  • その他
    ・投与を忘れた場合 次回投与までの期間が3日間（72時間）以上あれば、気づいた時点で直ちに投与し、その後はあらかじめ定めた曜日に投与してください。
    ・次回投与までの期間が3日間（72時間）未満であれば投与せず、次のあらかじめ定めた曜日に投与してください。
    ・なお、週1回の投与の曜日を変更する必要がある場合は、前回投与から少なくとも3日間（72時間）以上間隔を空けてください。
ADVERSE EFFECTS

副作用

悪心、嘔吐、下痢、便秘などの胃腸障害

食欲減退、頭痛など

臨床検査におけるリパーゼ・アミラーゼの増加

低血糖（稀）

強い空腹感、冷や汗、手の震え、めまいなど

急性膵炎（稀）

発熱、激しい上腹部痛、持続的なみぞおちや左あばらの下あたりの激痛など

胆嚢炎（稀）

発熱、皮膚や白目が黄色くなっている（黄疸）、右あばらの下あたりの痛みなど

上記の症状が見られた場合は、すぐに使用を中止して医療機関を受診してください。

  • ※1 本剤の成分に対してアレルギーのある方は、必ず事前に医師にご相談ください。
  • ※2 糖尿病性ケトアシドーシス、糖尿病性昏睡または前昏睡、１型糖尿病と診断された方は、必ず医師にご相談ください。インスリン製剤による速やかな治療が必須となるので、本剤の投与は禁忌とされています。
  • ※3 手術の予定がある場合や、重篤な感染症にかかった場合は、必ず医師にご相談ください。インスリン製剤による血糖管理が望まれるため、本剤の投与は禁忌とされています。
  • ※4 妊娠中や授乳中の方は、必ず事前に医師にご相談ください。本剤は妊婦又は妊娠している可能性のある女性への使用は原則避けることが推奨されています。また、治療上の有益性および母乳栄養の有益性を考慮しながら、授乳の継続または中止を検討することが推奨されています。
  • ※5 他に服用中の薬剤がある場合は、必ず医師にご相談ください。特に、本剤はセマグルチドを有効成分とするため、ウゴービなど他のセマグルチド製剤との併用はできません。また、薬の種類によっては低血糖などが起こる可能性があります。
  • ※6 本剤にはその他にも、使用に注意が必要な場合があります。ご使用前には必ず医師の診察と説明を受けてください。

未承認医薬品等本剤は、日本国内において2型糖尿病の治療薬としては厚生労働省により承認されていますが、肥満症や減量を目的とした効能・効果については未承認です。

国内の同一成分・性能の医薬品の有無本剤に含まれる有効成分「セマグルチド」を含む医薬品（例：オゼンピックなど）は、日本国内において2型糖尿病治療薬としては承認されていますが、肥満症を適応とした同一成分・性能を有する国内承認医薬品は存在しません。
諸外国における安全性情報米国FDAおよび欧州EMAにおいて、2型糖尿病治療薬として承認されており、食事療法・運動療法に加える補助療法として使用されています。
公的救済制度の適用本剤は、日本国内において承認された医薬品ですが、承認された適応・用法・用量を逸脱した使用や、美容目的での自由診療としての使用においては、医薬品副作用被害救済制度の対象外となる場合があります。
